Open Settings, enable Password protect form, enter a password, and save. Share the form link and password through separate channels when the information is sensitive. Anyone who knows both can open the form, so change the password if it is shared more widely than intended.
A shared password is not individual access control. Password protection does not identify which person used the password. Do not treat it as a substitute for a signed-in portal when you need per-person permissions.
