An Endpoint form gives your website or application a submission backend. You create and host the interface, then send its values to the submission endpoint.

The submission flow

  1. You create an Endpoint form in the dashboard.
  2. The form receives its own endpoint URL containing its Public Key.
  3. Your HTML form or application sends a POST request to https://api.surveyowl.co/v1/submit/YOUR_FORM_PUBLIC_KEY.
  4. The final URL segment identifies the destination form.
  5. The endpoint validates and stores the submission.
  6. The response appears in Submissions, and enabled notifications or integrations run.

What you get

  • a hosted submission endpoint;
  • storage and a response inbox;
  • CSV export;
  • email notifications;
  • Google Sheets, Notion, Airtable, Slack, Discord, Telegram, Zapier, and webhook connections;
  • file uploads, CAPTCHA and honeypot spam protection, and domain restriction.

What you provide

  • the visible form and its styling;
  • labels, client-side validation, and accessibility;
  • success and error messages when submitting with JavaScript;
  • the page where the form is displayed;
  • a CAPTCHA widget and token when CAPTCHA is enabled.

What an Endpoint form does not include

Endpoint forms do not have the visual builder, question blocks, themes, welcome or end screens, a hosted public link, Preview, Publish, the Share tab, or generated embed code. Those features belong to Hosted forms. If you do not want to build the page people fill in, use a Hosted form instead.
